You may print the forms from website to complete and mail to or fax the enrollment form and supporting documentation to Employee Benefits Division.  You may also print the open enrollment election form from the ARBenefits website at www.ARBenefits.org.

The best way to enroll or change your coverage during open enrollment is on the ARBenefits website.  Follow the registration instructions if you have not already registered on the website.

Employees that choose the Classic or Basic health insurance plan may wish to establish a Health Savings Account.  The DataPath HSA application is to be returned to the Personnel Department in the Administration Building.
